뇌하수체는 뇌 기저부 아래에 위치한 작은 콩알만한 크기의 샘 (gland)으로 줄기(stalk)'를 통해 뇌와 연결되어 있습니다. 뇌하수 체는 내분비계를 지배하는 샘이며 전신의 내분비(호르몬 분비) 샘의 기능을 조절합니다. 뇌하수체 선종은 뇌하수체에서 발생하는 가장 흔한 유형의 종양입니다. 대부분은 비기능성 (호르몬에 간섭 하지 않음) 이거나 무증상이며 '뇌하수체 우연종'으로 불립니다.
최근 WHO의 검토 결과, 뇌하수체 선종은 아래와 같이 재분류되었습 니다.
뇌하수체 부위의 종양에 대한 WHO 분류 제5판에 따라 '뇌하수체선종'이라는 용어는 '뇌하수체 신경내분비 종양 (Pituitary Neuroendocrine Tumour)', 줄여서 PitNET으로 명칭이 변경 됩니다.
- PitNET(2023년 1월부터 유효 함)에 대한 International
- Classification of Diseases for Oncology(ICD-O)의 행태코드 는 이제 "0"에서 "3"으로 변경 되어 양성에서 악성 질환으로 변경 되었습니다.
- 기존 뇌하수체 암종이라는 명칭도 전이성 뇌하수체 신경내분비종양(Metastatic PitNET) 으로 변경 되었습니다.
암보험 청구에 미칠 수 있는 영향
현황
암 진단 시 일시금이 지급되는 CI및 암보험의 경우 뇌하수체 선종에는 보험금이 지급되지 않습니다. 종양의 행태가 양성종양으로 분류되기 때문입니다. 그러나 종양이 전이된 경우 뇌하수체 암종으로 약관에 의해 지급 가능하며, 이 경우는 전체 뇌하수체 종양의 약 0.2%에 해당합니다.
새로운 분류
제5차 WHO 분류하에서는, 뇌하수체 선종은 이제 뇌하수체 신경내분비종양으로 명칭이 바뀌고 악성질환의 범주로 분류됩니다.
이로 인해 이제 대부분의 마켓에서사용하는 일반적인 암 정의에 기초한 CI 및 암보험의 암담보에서 보장 될 수 있는 가능성이 생겼습니다.
뇌하수체 종양의 역학은 흥미롭고 다양한 연구간에 상당한 차이를 보여줍니다. 미국의 대규모 인구통계 기반 연구1 에 따르면 전반적인 표준화 발생률은 연간 100,000명당 약 5건 이었습니다. 그러나 유병률은 인구통계학적 변수에 따라 크게 다릅니다. 한 연구에 따르면 영국에서 이 질병의 유병률은 인구 100,000명당 76-116명으로 약 1/1,000이지만 부검 시 뇌하수체 선종이 우연히 발견되는 경우는1.5%에서 최대 30%까지 높게 보고되었습니다.
이러한 재분류된 '악성' 뇌하수체 종양이 암담보에서 보장받을 수 있는 것으로 간주되는 경우, 보험 프라이싱 가정을 넘어 추가적으로 보험금이 청구되는 사례의 영향에 대한 평가를 신중하게 고려 해야 합니다.
뇌하수체 선종의 보험금 지급심사에 대한 퍼시픽 라이프재보험의 견해
1932년 이래로 Harvey Cushing 은 처음으로 뇌하수체 선종을 말단비대증의 원인으로 제안했습니다. 이 질병은 수술, 방사선요법 및 의학적 치료를 포함한 전통적인 치료법으로 쉽게 치료하거나 조절할 수 있는 양성종양으로 생각되었습니다.
대부분의 사례에서 우연히 발견 되고 증상이 나타나지 않으며 치료를 필요로 하지 않습니다. 이러한 경우 정기적인 검사만으로 충분합니다. 뇌하수체 암종(전이성 PitNET) 외에 뇌하수체 선종의 극히 일부만 공격적인 성향을 가지며, 이는 빠른 성장 속도와 최적화된 표준 치료에서의 불응성을 의미합니다.
암의 정의에서 모든 악성종양은 조직검사로 확진되어야 합니다.
임상적 치료가 필요하지 않거나 비침습적 치료가 유일한 치료방법인 경우, 이러한 종양은 조직검사를 통해 진단되지 않습니다.
요점은 WHO에 의해 악성으로 재분류 되었으나, 이 종양은 악성종양 행동양식 중에 다양한 성장패턴을 보이며, 대부분은 무증상으로 악성의 행동 양식을 보이지 않습니다.
WHO가 양성에서 악성으로 변경한 것은 새로운 병리적 소견을 나타내는 것이 아니라 명칭 변경으로, 기존에 양성 선종으로 여겨지던 것을 명백한 악성 종양으로 재분류해 명칭을 변경한 것입니다. WHO 재분류는 단순히 진단명이 수정되었다는 이유만으로 관련 위험이나 임상양상에 영향을 미치지 않습니다. 새로운 명칭이 부여되긴 했으나, 이들은 여전히 동일한 선종입니다.
따라서 조직학적으로 증명되지 않았거나 수술 또는 방사선 요법과 같은 특정 암 치료의 필요성이 없는 뇌하수체 종양은 CI 보험 약관에서 합리적으로 보장하고자 하는 하는 암 보장 범위를 벗어납니다.
특별히 일반적이지 않거나 '불명확한' 보험금지급 청구건은 퍼시픽라이프재보험에 문의하시길 바랍니다.
뇌하수체 선종의 언더라이팅
명칭 변경과 종양 범주의 재분류는 일반적인 진단에 영향을 미치지 않으므로, CI나 암상품을 심사할 때 뇌하수체 선종은 이제 잠재적으로 암 위험이 있다는 점을 고려해야 합니다.
비기능성인 경우(호르몬 균형에 영향을 미치지 않는)와 경증 종양의 경우에도, 뇌하수체 종양 보장제외(부담보) 적용을 권장합 니다.
성장이 빠르거나 치료에 대한 저항성이 있는 공격적인 사례의 경우 보다 신중하게 언더라이팅을 해야합니다.
우리의 현재 견해는 일반적인 진단 접근법과 뇌하수체 선종의 치료 기준이 WHO의 재분류로 인해 변하지 않는다는 점을 고려할 때, 소득보상보험 및 실손보험 상품에는 큰 영향을 미치지는 않을 것으로 생각됩니다.
당사의 언더라이팅 매뉴얼인 Ocean이 WHO의 변경 사항을 반영하여 업데이트 되었고 9월에 발간 예정이며 완료 후 다시 안내 드릴 것입니다.
그동안 언더라이터는 뇌하수체 선종 또는 PitNET의 병력이 있는 경우 CI 또는 암 전용상품을 평가 할 때 주의를 기울이고, 개별 사례에 대한 추가적인 언더라이팅은 당사에 문의할 것을 권고합니다.
